I, [full name of citizen A.], holder of passport series ____ № _____________, issued by _______________________________ "____" _____________ ______ year, registration number of the taxpayer's account card ____________, I live in Irshava, Transcarpathian region [address], in accordance with the oral power of attorney agreement and in compliance with the requirements of Article 1007 of the Civil Code of Ukraine, have issued this power of attorney to [full name of citizen B.] (hereinafter referred to as the Representative), registered in the State Register of Individuals Payers of Taxes and Other Obligatory Payments with the identification number ____________, residing in the city of Irpin, Kyiv region [address], for representation before the relevant customs authorities and their officials with the right to:
- act as a declarant of a vehicle purchased by me abroad - a passenger car PRADO LAND CRUISER 150, manufactured in 2022, chassis No. WCM2201671Z366034;
- submit a customs declaration and all documents necessary for customs control and clearance to the customs authority that will carry out the vehicle clearance;
- be present during the customs clearance of the vehicle presented for such clearance;
- receive documents issued in connection with this power of attorney from the customs authorities, including certificates, references, extracts and other documents.
The Representative is also authorised to represent my interests before the territorial service centres of the Ministry of Internal Affairs of Ukraine (formerly the traffic police / MREO) and other authorised bodies with the right to:
- submit applications and other documents on my behalf;
- receive a temporary registration certificate, a vehicle registration certificate, their duplicates, licence plates, including transit plates, and other registration documents;
- carry out state registration, re-registration, deregistration and changes to the registration details of the vehicle;
- pay taxes, fees, administrative charges and other mandatory payments related to the customs clearance and registration of the car.
The Representative also has the right to represent my interests before the National Police of Ukraine, insurance companies, notaries, service stations, enterprises, institutions and organisations of any form of ownership and subordination, if this is connected with the use, transport, registration, insurance, repair and storage of the car; to submit and sign applications, explanations, requests, acts and notices, to receive certificates, copies and other documents, and to perform all other legally significant actions related to this power of attorney.
This power of attorney is issued until "___" ____________ 20__ year without the right to transfer the powers to other persons. The content of Articles 244-248 of the Civil Code of Ukraine and the rules on the state registration of vehicles have been explained to me.
