I, [full name of citizen A.], holder of passport series ____ No. _____________, issued by _________________________ “___” ____________ _____ year, taxpayer registration number ____________, residing at ________________________, acting under an oral mandate agreement and in accordance with Article 1007 of the Civil Code of Ukraine, hereby authorise [full name of citizen B.], who resides at ________________________, to represent my interests in connection with the truck crane that belongs to me as private property, a Liebherr LTM 1055, model LTM 1055, vehicle type - truck crane - over 20 t-S, year of manufacture 2022, yellow, chassis (body, frame) No. _________________, registration number ___, evidenced by vehicle registration certificate series _____ No. ________________, issued by the territorial service centre of the Ministry of Internal Affairs of Ukraine “___” ________________ ____ year, with the right to:
- represent my interests in the territorial service centres of the Ministry of Internal Affairs of Ukraine, the National Police of Ukraine, insurance companies, notaries, technical service, repair, storage and towing companies, as well as in other enterprises, institutions and organisations regardless of their form of ownership or subordination;
- submit and sign applications, explanations, motions, questionnaires, acts, notices and other documents, and receive certificates, conclusions, copies, duplicates, the vehicle registration certificate, a temporary registration card, number plates, including transit plates, and other documents related to the truck crane;
- carry out state registration, re-registration, deregistration and changes to the registration data of the truck crane, undergo the state technical inspection, arrange maintenance, repair, diagnostics, storage, towing and transportation of the truck crane, including while driving it during its movement, provided that the attorney holds the appropriate driving category;
- order and pay for services related to the operation of the truck crane, as well as pay taxes, fees, administrative and other mandatory payments, including payments related to its registration and maintenance;
- enter into civil liability insurance contracts for the owner of the vehicle and other insurance contracts, receive insurance policies, certificates and other documents related to an insured event;
- perform all other legally significant actions related to the proper operation and maintenance of the truck crane.
This power of attorney is issued without the right to dispose of the truck crane and without the right to delegate these powers to third parties, and remains valid until “___” ____________ 20__ or until revoked in the manner prescribed by civil law.
