M., a notary of the Kyiv City Notarial District (6-a Mykhailivska Street, apartment 3, Kyiv), whom I have chosen as my notary independently, pursuant to Article 1007 of the Civil Code of Ukraine, hereby issue this power of attorney to [full name of citizen B.], residing in the city of Kyiv (address), with the following powers:

For this purpose, [full name of citizen B.] shall have the right to represent my interests before territorial service centres of the Ministry of Internal Affairs, notaries, customs authorities, insurers, business entities engaged in commission trade in vehicles and numbered components, business entities performing maintenance and repair of vehicles, and any other enterprises, institutions and organisations irrespective of their subordination and form of ownership, in all matters without exception relating to the use, operation, disposal and alienation (sale and exchange) of the vehicle, its re-equipment (change of vehicle type and model, colour, replacement, in accordance with the established procedure, of the engine, body and other parts), repair, mandatory technical control, state registration, re-registration, deregistration, amendment of registration details, and determination at his or her discretion of the place where the vehicle is kept; to file and receive all necessary certificates and documents, including applications, the certificate of registration of the vehicle, a duplicate registration document in case of loss, registration plates, temporary registration certificates, acts, extracts and other documents; to enter into civil-law contracts on my behalf concerning the disposal and use of the vehicle (sale, exchange, commission, lease, loan for use, pledge, insurance, maintenance and repair, etc.), except for a deed of gift, determining at his or her discretion the price and other terms, including optional terms, in such contracts, and to receive the sums of money due to me under such contracts; to enter into contracts of compulsory motor third-party liability insurance, receive the policy of compulsory motor third-party liability insurance, and receive insurance payments and compensation; in the event that the vehicle owned by me is damaged as a result of a road traffic accident, to enter into agreements for compensation of damage on my behalf, receive the compensation due to me, enter into contracts for maintenance and repair of the vehicle, visually supervise the performance of work under such contracts, receive from the contractor documentary confirmation of the type and scope of the maintenance or repair performed on the vehicle, pay on my behalf within the established time limits and in the established amounts taxes, fees, state duty, administrative charges and any other mandatory payments related to this vehicle, and also perform any and all other legally significant acts connected with this power of attorney.
For the exercise of the powers granted, the representative shall have the right to use and operate the vehicle for its intended purpose.
This power of attorney is issued for the period until "___" ____________ 20__ inclusive, with the right to delegate these powers to other persons by way of substitution, and with the right to use the vehicle outside Ukraine.
