in accordance with the oral power of attorney agreement and in compliance with the requirements of Article 1007 of the Civil Code of Ukraine
I hereby give this power of attorney to [full name of citizen B.], who lives in Kyiv [address], for the registration of my ownership of the participation in the construction of the facility under the Agreement No. __________ dated __________ ___, concluded with the limited liability company [name] apartment located at: Kyiv region, Kyiv-Svyatoshinsky district, Shpitky village [address], and to the apartment ____ in the house __, entrance __, floor __, owned by me on the basis of the Agreement No. ______________ of participation in the construction of the object from "_____" _________________ ___________ year.
For which the representative is authorized to represent my interests:
 in the executive authorities and local self-government bodies, their subordinate bodies and structural subdivisions, etc;
 in the relevant bureau of technical inventory and registration of ownership of real estate regarding the registration of my ownership of the above real estate;
 in state and non-state bodies and communication institutions;
 in housing and maintenance authorities, before insurers, appraisers;
 in the internal affairs authorities regarding the registration of my place of residence in the apartment [address] of the village of Chaiky;
 in the relevant energy, water, gas supply authorities, etc;
 before any other enterprises, institutions and organizations regardless of their subordination, ownership and affiliation, as well as individuals.
In this regard, I grant the representative the right:
 free access to the above-mentioned apartments;
 to submit on my behalf the necessary documents, including statements related to the execution of this power of attorney and the need for which will be caused by my interests as the owner of the property
 to receive documents due to me, including title documents for each of the specified apartments, certificates, duplicate documents, repeat documents, as well as any other documents relating to my rights and interests (decisions, permits, certificates, etc.) as the owner of the apartments, order the production and receive design, technical and other documentation and documents;
 pay the payments due from me;
 sign documents, including applications, sign for receipt of documents, as well as coordinate and resolve all other issues that may arise in the course of exercising the powers.
The power of attorney is issued without the right to delegate powers to other persons and is valid until September twelve, two thousand twenty-eight.
The content of Articles 240, 244, 247-248 of the Civil Code of Ukraine has been explained to me by a notary.
