registered in the State Register of Individuals Payers of Taxes and Other Obligatory Payments with the identification number ____________, who lives in the city of Krasnoarmiysk, Donetsk region [address],
TO REPRESENT MY INTERESTS
regarding the apartment No. ___ in the house No. __ on Velyka Kytayivska Street in Kyiv, which belongs to me on the right of private ownership:

 in the executive authorities and local self-government bodies, their subordinate bodies and structural subdivisions, in the Main Department of Housing of the Kyiv City State Administration, state architectural control bodies, etc., including for the approval of all issues without exception and obtaining permission, as well as any other documents and documentation for the conversion, redevelopment of the said apartment;
 at the Municipal Enterprise "Kyiv City Bureau of Technical Inventory and Registration of Ownership of Real Estate Objects";
 in the bodies of internal affairs, including the State Fire Protection Service in Kyiv and its territorial subdivisions;
 in the bodies and institutions of the State Sanitary and Epidemiological Service;
 in state and non-state bodies and institutions of communication;
 in housing and maintenance authorities, design organizations, insurers, appraisers;
 in the relevant energy, water, gas supply authorities, etc;
 before any other enterprises, institutions and organizations regardless of their subordination, ownership and affiliation, as well as individuals on all issues without exception related to the exercise of the powers granted under this power of attorney.
In this regard, I authorize the representative to:
 to submit documents and applications on my behalf;
 to receive documents, duplicates of documents, repeated documents, as well as any other documents relating to my rights and interests (decisions, permits, certificates of ownership, etc.), to order and receive the prepared design and technical documentation
 to enter into civil law contracts on my behalf with housing and maintenance authorities, energy, water, gas and communication organizations, design organizations, etc., as well as contracts for making amendments and additions to them;
 pay monetary amounts due from me both under civil law contracts and in connection with the ordering and production of documents and documentation;
 sign documents, including applications, and sign for receipt of documents.
According to this power of attorney, the representative is also entitled to use and dispose of the said apartment, for which purpose he/she has the right to sell it, lease it, pledge it as security for fulfillment of my own obligations, represent my interests in notary bodies, housing and maintenance authorities, the Kyiv City Bureau of Technical Inventory and Registration of Ownership of Real Estate Objects Municipal Enterprise when ordering and receiving a certificate of characteristics, tax authorities, territorial authorities for citizenship, immigration and registration of individuals in connection with registration of residence or deregistration of residence of persons registered in the apartment, to submit on my behalf the necessary documents, including statements relating to the execution of this power of attorney and the need for which will be caused by both my interests as the owner of the apartment, to fill in, sign and submit declarations, receive any certificates and other documents issued in my name, sign sale and purchase agreements on my behalf, enter into other agreements, including those related to the use of heat, water, electricity and communication services, for the maintenance of the building and adjacent territory, to sign for me on other documents and in their receipt, to coordinate all issues that may arise in the course of exercising the powers, to pay payments due from me, including if they are provided for by the terms of the sale and purchase agreements, requirements of the current legislation, to receive money due to me, and to perform all other legally significant actions related to this power of attorney.
By authorizing [full name of citizen B.], I grant him/her the right to enter into agreements on the alienation of the apartment owned by me at any price, with the determination of terms and any other essential conditions at his/her own discretion.
The power of attorney is issued without the right to delegate powers to other persons and is valid until January thirtieth, two thousand twenty-seven.
