POWER OF ATTORNEY

I, [full name of citizen A.] (born "__" _____________ ___ year; on the date of issuance of the power of attorney - passport _____, issued by ___________________________________ "__" ____________ ___ year; taxpayer registration number (RNOKPP) - ___), residing in Kyiv [address], fully aware of the significance of my actions and acting voluntarily, in accordance with Articles 244-250 of the Civil Code of Ukraine, hereby authorise [full name of citizen B.], who lives in Kyiv [address], to accept for me as a gift from [full name of citizen V.] (born "___" ____________ ___ year; passport _______, issued by ___________________________________________________ "__" ___________ ___ year; taxpayer registration number (RNOKPP) - _________; registration of residence - Kyiv [address]) the part of apartment No. __, located in building No. __, on Yevhen Konovalets Street in Kyiv, which belongs to him on the right of private ownership, and to perform all actions necessary for the notarisation of the gift agreement and the state registration of my ownership rights to the gifted property.
For this purpose, the attorney is entitled to:
- represent my interests before a notary, a centre for administrative services (CNAP), the state registrar of rights to immovable property and their encumbrances, the BTI or any other technical inventory provider, local self-government bodies, residence registration authorities, tax authorities, OSBB, the managing company, housing maintenance organisations, utility providers and any other institutions or organisations if needed to perform this power of attorney;
- submit, sign and receive applications, requests, consents, certificates, extracts, information certificates, a technical passport, technical documentation, BTI documents, acts, receipts, preliminary agreements, the gift agreement, the handover and acceptance act and any other documents needed for the notarisation of the gift and state registration of ownership;
- obtain consents of co-owners, spouses and any other documents if they are required by law;
- sign for me, including the gift agreement, applications for state registration of ownership, handover and acceptance acts, applications for corrections or amendments, and other supporting documents;
- file documents for state registration of ownership and receive an extract from the State Register of Rights to Immovable Property, the registration decision, a refusal notice or any other document issued after the applications are considered;
- pay notarial, registration, technical, valuation, tax and other mandatory charges, if they arise under the law, connected with the gift transaction and state registration;
- if needed, file applications for residence registration in the gifted apartment and enter into and sign related agreements with the condominium association, the managing company and utility providers;
- challenge refusals, actions or omissions of officials and perform any other acts necessary for the full performance of this power of attorney.
This power of attorney is issued without the right to delegate powers to third parties. It shall be valid until "___" ____________ 20__ unless revoked by me earlier. It remains valid if the representative changes their residential address and/or passport.
The content of Articles 244-250 of the Civil Code of Ukraine and the provisions of the Tax Code of Ukraine concerning the taxation of income received as a gift have been explained to me by a notary.
