POWER OF ATTORNEY

I, [full name of citizen A.], holder of passport series ____ No. _____________, issued by _______________________________ on "____" _____________ _____ year, taxpayer registration number ____________, residing in Kyiv [address], acting voluntarily and in accordance with Articles 244-250 of the Civil Code of Ukraine, hereby authorise [full name of citizen B.], who resides in Kyiv [address], to represent my interests as a sole proprietor and to conclude on my behalf with the Joint Stock Company "Insurance Company PZU Ukraine" a lease agreement for the non-residential premises located in Poltava [address], for the term and on the terms known to the attorney, and to perform all actions necessary for the signing, performance, renewal, amendment, termination and, if required by law, state registration of such agreement.
For this purpose, the attorney is entitled to:
- represent my interests before a notary, a centre for administrative services (CNAP), the state registrar of rights to immovable property and their encumbrances, the BTI or any other technical inventory provider, local self-government bodies, executive bodies, tax authorities, banks, utility providers, residence registration authorities and any other institutions or organisations if needed for the performance of this power of attorney;
- submit, sign and receive applications, requests, certificates, extracts, a technical passport, technical documentation, acts, receipts, decisions, agreements, supplementary agreements and any other documents needed to conclude and perform the lease agreement;
- sign on my behalf the lease agreement, supplementary agreements, handover and acceptance acts, inspection acts, applications for extension, amendment or termination of the agreement, and other supporting documents;
- agree the lease term, rent amount, payment procedure, indexation, security deposit, conditions of use of the premises, access, repairs and other material contract terms;
- receive and transfer money, rent, deposit, reimbursement of expenses, utility compensation, penalties or any other payments connected with the agreement;
- pay notarial, registration, technical, banking, tax and other mandatory charges, if they arise under the law;
- obtain extracts, certificates and other documents needed to arrange the lease, including BTI documents, ownership documents and information from the State Register of Rights to Immovable Property, if required by law or the contract terms;
- if a dispute arises that cannot be settled by negotiation, represent my interests in all courts with the full procedural rights of a claimant, defendant or third party, including the right to bring claims, file responses, objections and motions, lodge appeals and cassation appeals, amend the grounds or subject matter of a claim, withdraw a claim, admit a claim, conclude a settlement, and participate in the proceedings at any stage;
- conduct my matters before the state enforcement service regarding the enforcement of court and other decisions, submit enforcement documents, receive property or money awarded, inspect enforcement case files, file motions, challenge the actions or omissions of the state enforcement officer, obtain duplicate enforcement documents, request renewal of the time limit for presenting an enforcement document for execution, and challenge property valuation;
- conduct matters before other bodies, including law enforcement agencies, tax authorities, enterprises, institutions and organisations regardless of subordination and form of ownership, as well as before sole proprietors and individuals, submit and sign any necessary documents and applications on my behalf, and receive decisions, permits, acts and other documents;
- perform any other actions necessary for the proper and full performance of this power of attorney.
This power of attorney is issued without the right to delegate powers to third parties and shall remain valid until the thirtieth day of June, two thousand twenty-seven.
The content of Articles 244-250 of the Civil Code of Ukraine, as well as the provisions of the Tax Code of Ukraine and the legislation on the state registration of rights to immovable property, have been explained to me by a notary.
