in accordance with the oral power of attorney agreement and in compliance with the requirements of Article 1007 of the Civil Code of Ukraine
I hereby authorize

to represent my interests
in the relevant authorized enterprises, institutions and organizations on obtaining a duplicate of the title document (or a repeat document, its archival copy) for the apartment at No. __ in the house at No. __ on Kikvidze Street in Kyiv and registering the ownership of the said real estate.
For this purpose, the representative is also authorized to represent my interests:
 in executive authorities and local self-government bodies, their subordinate bodies and structural units, as well as in notary bodies, the state notary archive, other archival institutions, etc;
 in housing and maintenance organizations;
 in the relevant energy, water, gas supply authorities, etc;
 before any other enterprises, institutions and organizations regardless of subordination, ownership and affiliation.
In this regard, I authorize the representative to:
 to submit on my behalf the necessary documents, including statements related to the execution of this power of attorney and the need for which will be caused by both my interests as the owner of the property and the requirements of the applicable law;
 to receive documents, certificates, duplicate documents, repeated documents, as well as any other documents relating to my rights and interests (decisions, permits, certificates, etc.) due to me;
 pay the amounts of money due from me (state duty, fees, other payments);
 to sign for me, including for obtaining a duplicate of a document of title, sign other documents, including applications, as well as to agree and resolve all other issues that may arise in the course of exercising the powers.
The power of attorney is issued without the right to delegate powers to other persons and is valid until November twenty-second, two thousand twenty-five.
