POWER OF ATTORNEY

I, [full name of citizen A.], holder of passport series ____ No. _____________ issued by _______________________________ on "____" _____________ _____, taxpayer registration number ____________, residing in Kyiv [address], acting voluntarily and fully aware of the meaning of my actions, and pursuant to Articles 244-250 of the Civil Code of Ukraine, hereby authorise [full name of citizen B.], who resides in Kyiv [address], to represent my interests in matters related to obtaining a duplicate of a title document (or a repeat document, or an archival copy) for apartment No. __ in building No. __ at [address], as well as for the state registration of ownership rights to the said real estate.
For this purpose, the representative is also authorised to represent my interests:
- before executive authorities and local self-government bodies, their subordinate bodies and structural units, Centres for Administrative Services (CNAP), state and private notaries, the State Notarial Archive, other archival institutions, state registrars of rights to immovable property and their encumbrances, BTI, residence registration authorities, housing and maintenance bodies, the relevant electricity, water and gas supply bodies, and any other enterprises, institutions and organisations, regardless of subordination, ownership or affiliation, if this is needed to obtain the duplicate document and register ownership rights;
- to submit on my behalf the necessary documents, including applications, requests, petitions, copies, extracts and other materials required for the performance of this power of attorney;
- to receive documents, certificates, duplicate documents, repeat documents, archival copies, extracts from the DRRP, and any other documents relating to my rights and interests, including decisions, permits, certificates and other supporting documents;
- to pay any sums due from me, including state duty, administrative fees, charges for issuing or requesting documents, archive fees and any other payments provided for by law;
- to sign for me, including for the receipt of the duplicate title document, to sign applications, acts, receipts and other documents, and to agree and resolve any other issues that may arise in the course of exercising these powers.
This power of attorney is issued without the right to delegate powers to other persons and remains valid until the twenty-second day of November, two thousand twenty-five.
The content of Articles 244-250 of the Civil Code of Ukraine, as well as the legislation on the state registration of rights to immovable property, the activities of state registrars of rights to immovable property and their encumbrances, the State Register of Rights to Immovable Property, archival institutions, and the procedure for issuing duplicates and archival copies, has been explained to me by a notary.
