in accordance with the requirements of Art. 244 and Art. 1007 of the Civil Code of Ukraine on the basis of an oral power of attorney concluded with the attorney,
in the presence of Alla Mykhailivna Kvitka, a notary of the Kyiv City Notary District, whom I have chosen as a notary for this transaction on my own and to whom I voluntarily applied at the location of her workplace (Kyiv, Mykhailivska St., 6A, apartment 3), by this power of attorney

1) in any institutions, organizations and enterprises, regardless of their organizational form and subordination, as well as to individuals on issues related to the execution of documents, management and lease/lease of real estate owned by me on the right of private property, namely, the apartment No. __, located in the house No. _, located on Pravdy Avenue in Kyiv;
2) in OJSC "STATE SAVINGS BANK OF UKRAINE" (including the Shevchenkivskyi branch, branch No. ___) - for the management and disposal of account No. ___ on my behalf and in my interests.
For this purpose, [full name of citizen B.] is entitled to:
 have free access to the above real estate and account;
 to prepare all documents for the lease/rental of the above real estate without exception;
 in cases known to him/her, to carry out repairs to the property, with the right to draw up relevant documents (permits, approvals, consents, etc.);
 to carry out the entire range of actions provided for by the current legislation of Ukraine for me as a member of a housing cooperative who has purchased the apartment, regarding the legal registration of ownership of the above apartment in the Housing Cooperative [name] (including obtaining the necessary certificates from the housing cooperative, obtaining a certificate of ownership of the said apartment, registering the ownership of the apartment with the technical inventory authorities, etc;
 to represent my interests in connection with the assignment in any institutions or enterprises, regardless of organizational form and subordination, on issues related to ordering and obtaining the documents I need (including in local governments, executive bodies, housing and communal authorities, any other organizations and institutions of the city of Kyiv (including, but not limited to the following list: district sanitary and epidemiological station, state fire protection authorities, gas supply authorities, the relevant department of urban planning, architecture and regulation of land relations of the district, the relevant structural unit of AEK Kyivenergo or other enterprises/institutions equivalent in terms of their powers, the State Archive, in the bodies of communication, water supply, labor protection, etc. - on issues related to the proper execution of the power of attorney and whose competence is related to the resolution of issues of my interest, as well as in banking institutions, notary, technical inventory, internal affairs, etc. - when such a need arises, etc;)
 to provide arguments and considerations on all issues arising in the course of the assignment, as provided for by law;
 supervise the proper fulfillment of the terms of the concluded lease/lease agreements (including maintenance of the apartment, payment of due fees, payment of the lease/lease agreement, etc;)

 in case of violation of the terms of lease/lease agreements - to raise the issue of termination of such agreements, including in court (with all procedural rights and obligations granted by law to the plaintiff, defendant, third party in the process, including the right to file a claim (administrative claim), recognize the claim (administrative claim) in whole or in part, file objections to the claim, withdrawal of the claim at any time before the end of the trial (including in the court of appeal or cassation), recognition of the claim in whole or in part, change of the grounds or subject matter of the claim, increase or decrease of the amount of the claim, withdrawal of the claim, filing a counterclaim, conclusion of an amicable agreement (reaching reconciliation at any stage of the administrative process), submission of enforcement documents to the state executive service on my behalf, receipt of the awarded property or money);
 to sign for me, in particular, but not exclusively, to sign applications (including claims), applications, agreements on the transfer of rights to use real estate (lease, rent), amendments to the concluded agreements, termination thereof on the terms and conditions that the representative is previously aware of and do not require any additional approval from me;
 to receive documents drawn up in my name (with my participation), including (but not limited to): a certificate of ownership, a certificate of characteristics and/or an extract from the Register of Ownership of Real Estate, which is referred to in the power of attorney, extracts from state and unified registers, technical documentation, if necessary, certificates of payment of share contributions, the condition of the apartment, the number of persons registered in the living space, copies of agreements intended for me, etc;
 to receive money due to me from the concluded lease/lease agreements;
 to receive any amounts from the above account (including interest accrued on the deposit amount) through the Bank's cash desk without any restrictions on their amount;
 to convert currencies on the terms and conditions previously agreed upon by us;
 to receive certificates or other documents on the availability of funds, interest (accruals) on them or the movement of these amounts;
 to replenish the account (including by converting currencies);
 in cases known to her from our previous agreements - to amend the agreement concluded with the Bank, to conclude additional agreements on the extension of the agreement in respect of the account specified in the power of attorney (with the performance of all necessary legal actions);
 pay for the services rendered on my behalf and/or pay state duty, income tax or other necessary payments (including using funds held on the above account);
 perform other actions provided for by the current legislation of Ukraine for such powers of attorney and which are necessary and appropriate for the correct and efficient execution of the assignment, even if they are not directly stated in the text of the power of attorney.
The power of attorney is issued without the right of the representative to reassign the powers granted to third parties, and shall remain valid until JUNE THIRTEENTH, TWO THOUSAND TWENTY-FIVE, unless terminated by me in accordance with the procedure established for that purpose, which the representative will be notified of in writing.

The content of Articles 240-245, 247-250 of the Civil Code of Ukraine regarding the concept of a power of attorney, its form, terms, assignment, termination of representation by power of attorney, revocation of a power of attorney and refusal of a representative to perform actions specified in the power of attorney, as well as Articles 60-74 of the Family Code of Ukraine regarding the status of marital property and the conditions for its disposal by a notary has been explained to me.
