POWER OF ATTORNEY

I, [full name of citizen A.], passport ___, issued by _______________________________ on "__" ____________ ___ year, taxpayer identification number ___, residing (registered) in Kyiv [address], acting voluntarily and fully aware of the meaning of my actions, pursuant to Articles 244-250 of the Civil Code of Ukraine, hereby authorise [full name of citizen B.], who resides in Kyiv [address], to represent my interests in relation to the preparation of documents, management and rental of apartment No. __ in building No. __ on European Union Avenue, formerly Pravdy Avenue, in Kyiv, which belongs to me by right of private ownership, as well as in relation to account No. ___ held at the banking institution where this account is opened.
For this purpose, the attorney may:
- have free access to the apartment, inspect it, receive and hand over keys, record the condition of the premises and prepare inspection reports when needed;
- enter into, amend and terminate lease agreements, preliminary agreements, addenda, handover and return acts, and any other documents connected with renting out the apartment;
- agree on the lease term, rent, payment procedure, advance payment, security deposit, reimbursement of utility costs and other contract terms;
- represent my interests before a notary, a centre for administrative services, the state registrar of rights to immovable property, a district state administration or district military administration, local self-government bodies, the technical inventory office (BTI), the condominium association (OSBB), the managing company, utility providers, tax authorities and any other institutions if needed for this power of attorney;
- submit, sign and receive applications, explanations, certificates, acts, invoices, receipts, extracts, information certificates, technical documents and other documents connected with the use, maintenance and rental of the apartment;
- take all actions required to register ownership of the apartment through a housing construction cooperative, obtain cooperative certificates, file documents for state registration of ownership and receive extracts and other registration documents;
- organise, order and supervise routine repairs without redevelopment, sign work contracts and accept the completed work;
- dispose of account No. ___ at the banking institution: submit and sign applications and payment orders, order transfers, receive and deposit funds, obtain statements, certificates and information on account movements, change the terms of service, extend the agreement and carry out currency conversion if this is allowed by the bank;
- receive rent, advance payments, security deposits, expense reimbursements, insurance compensation and other amounts related to the apartment agreements;
- pay notarial fees, administrative charges, utility bills, taxes, duties, bank commissions and other mandatory payments if such payment is required for the performance of this power of attorney;
- take the matter to court and represent my interests in disputes related to the use, maintenance and return of the apartment if the lease terms are breached;
- perform any other legally significant acts necessary for the proper management of the apartment and the account, within the limits of the current legislation of Ukraine.
This power of attorney is issued without the right to alienate the apartment specified herein or any other property or property rights belonging to me, and without the right to delegate powers to third parties. It shall be valid until "___" ____________ 20__ unless revoked by me earlier. It remains valid if the representative changes their residential address and/or passport.
The content of Articles 244-250 of the Civil Code of Ukraine, Articles 60-74 of the Family Code of Ukraine concerning the regime of spouses' joint property and the requirements of the Tax Code of Ukraine has been explained to me by a notary.
