POWER OF ATTORNEY

I, [full name of citizen A.] (born "__" ___________ ___ year; on the day of issuance of the power of attorney - passport _________, issued by _______________________________ "__" _________________ ___ year; registration number of the taxpayer's account card - ___), residing in Kyiv [address],
acting voluntarily, fully aware of the meaning of my actions and pursuant to Articles 244-250 of the Civil Code of Ukraine, by this power of attorney I authorise [full name of citizen B.], who lives in Kyiv [address],
to represent my interests in ordering, preparing and obtaining the technical passport, certificate of characteristics, explication, floor plan and other technical documentation for my apartment at: Kyiv [address].
For this purpose, the representative is entitled to:
- at BTI, administrative service centres, before state and private notaries, state registrars, local self-government executive bodies, residence registration bodies, housing maintenance organisations, homeowners' associations, managing companies, communal enterprises, archive institutions and other bodies and organisations, if this is necessary for issuing the technical passport;
- submit, sign and receive applications, requests, petitions, forms and other documents required for technical inventory, production of the technical passport, floor plan, explication and other technical documentation;
- receive the technical passport, certificate of characteristics, copies, floor plan, explication, other documents related to the technical passport and the results of technical inventory;
- pay the administrative fee, state duty, notarial, archive, registration and other mandatory charges;
- perform any other actions necessary for the preparation of the technical passport and the related documents.
The power of attorney is issued without the right to delegate the powers granted to third parties and is valid until the twenty-fifth day of January, two thousand twenty-eight, unless revoked earlier in accordance with the procedure established for that purpose.
The content of Articles 244-250 of the Civil Code of Ukraine, as well as the legislation on the technical inventory of immovable property, the preparation of technical documentation and the state registration of rights to immovable property has been explained to me by a notary.
