acting voluntarily, understanding the meaning of my actions and guided by Articles 244-250 of the Civil Code of Ukraine, by this power of attorney I hereby authorize [full name of citizen B.] (as of the date of issuance of the power of attorney - passport ________, issued by ______________________________________________________ “__” ___ ___ year), who lives at the address: Kyiv [address], registration number of the taxpayer's account card _________,
to be my representative in the relevant institutions, enterprises and organizations regardless of their subordination and form of ownership on matters related to the preparation, coordination, ordering and obtaining of documents for the repair, redevelopment, reconstruction, re-equipment and other technical formalities relating to the real estate objects owned by me on the right of private ownership, namely:
a) APARTMENT No. __, located in the building No. __, located on Khoryva Street in the city of KYIV;
b) APARTMENT No. __, located in the building No. __, located on Khoryva Street in the city of KYIV;
c) APARTMENT No. __, located in the building No. __, located on Khoryva Lane in the city of KYIV.
For this purpose, I grant the representative the right to:
- have free access to the specified objects;
- prepare, submit, sign and receive all documents relating to the repair, redevelopment, reconstruction, re-equipment and technical processing of the objects, including project, technical and other documentation;
- apply to administrative service centres (CNAP), bureau of technical inventory (BTI), district state administrations in Kyiv, executive bodies of local self-government, state registrars of rights to immovable property and other subjects of state registration of rights, state and private notaries, state architectural and construction control bodies, archive institutions, residence registration bodies, OSBB, managers, balance holders, municipal enterprises and utility providers, if this is needed for document processing;
- submit, sign and receive applications, requests, notices, petitions, explanations, schemes, plans, declarations, acts, conclusions, technical conditions, approvals, permits, decisions, extracts and other documents required to perform this power of attorney;
- order and obtain technical passports, descriptive certificates, floor plans, explications, registry extracts, extracts from the territorial community register, certificates of registered persons, copies, duplicates and title documents, if they are needed to formalise the results of the works or the state registration of changes;
- enter into and sign contracts, applications and orders necessary for technical inventory, production of documentation and formalisation of its results;
- obtain documents related to commissioning the objects after reconstruction or other works, if such a procedure is required by law;
- represent my interests when submitting and receiving documents for the state registration of changes in respect of the real estate objects after the works are completed;
- pay administrative fees, state duty, notarial, registration, archive and other mandatory charges;
- sign necessary documents for me;
- perform any other actions necessary for the full performance of this power of attorney.
The power of attorney is issued without the right to delegate powers to third parties and without the right to alienate or otherwise dispose of the real estate, and is valid until the twenty-second day of February, two thousand twenty-nine, unless revoked earlier in accordance with the procedure established for that purpose, which I will notify the representative of in writing.
The content of Articles 244-250 of the Civil Code of Ukraine regarding the concept of a power of attorney, its form, term, delegation, termination of representation by power of attorney, revocation of a power of attorney and refusal of a representative to perform the actions specified in the power of attorney has been explained to me by a notary.
