POWER OF ATTORNEY

acting voluntarily, understanding the meaning of my actions and guided by Articles 244-250 of the Civil Code of Ukraine, by this power of attorney I hereby authorize [full name of citizen B.], who lives in Kyiv [address],
to be my representative in respect of apartment No. __ in building No. ___, located on Khoryva Street in Kyiv, on all matters related to its use, maintenance, repairs, technical service, conclusion of contracts and receipt of documents, namely:
- to apply to administrative service centres (CNAP), bureaux of technical inventory (BTI), state registrars of rights to immovable property, executive bodies of local self-government, district state administrations in Kyiv, residence registration bodies, OSBB, managers, balance holders, housing-maintenance organisations, utility companies, energy, water, heating and gas supply bodies and communications bodies, cable television network operators, and, if necessary, internal affairs bodies and other bodies to protect my rights as the owner of the apartment;
- to submit on my behalf applications, documents, explanations, requests, petitions and other documents necessary to perform this power of attorney, and to receive any certificates, permits, decisions, duplicates, repeated documents, technical documentation and other documents related to the apartment;
- to sign for receipt of documents, coordinate and resolve all issues that may arise during the exercise of the powers granted, and pay the charges and fees due from me;
- to enter into contracts with OSBB, managers, housing-maintenance organisations, communications bodies and enterprises, heat, water, electricity and gas supply bodies, as well as organisations servicing cable television networks;
- to enter into works contracts with legal entities and individuals if repair works are carried out in the apartment, to prepare and agree the estimate required for that purpose, to monitor the progress of repair works, and to pay the cost of the works performed;
- to resolve issues relating to the installation, replacement and accounting of water, gas, electricity and other metering devices;
- to obtain certificates of registered persons, extracts from the territorial community register, technical documentation, acts, decisions, approvals and other documents if they are needed to exercise this power of attorney;
- to perform all other legally significant actions related to this power of attorney.
The power of attorney is issued without the right to delegate powers to other persons and is valid until the twenty-second day of July, two thousand twenty-nine, unless revoked by me earlier in accordance with the established procedure, of which I will notify the representative in writing.
The content of Articles 244-250 of the Civil Code of Ukraine regarding the concept of a power of attorney, its form, term, delegation, termination of representation by power of attorney, revocation of a power of attorney and refusal of a representative to perform the actions for which it was issued has been explained to me by a notary.
