I am aware of the general requirements, compliance with which is necessary for the validity of the transaction, fully realizing the significance of my actions and in accordance with my free will, which corresponds to my inner will as a party to this transaction,
in accordance with and in compliance with the requirements of Art. 244 and Art. 1007 of the Civil Code of Ukraine on the basis of an oral power of attorney concluded with the attorney,
in the presence of Alla Mykhailivna Kvitka, a notary of the Kyiv City Notary District, whom I have chosen as a notary for this transaction on my own and to whom I voluntarily applied at the location of her workplace (Kyiv, Mykhailivska St., 6A, apartment 3), by this power of attorney

in the relevant institutions, enterprises, organizations regardless of their subordination and organizational form (including local governments, executive bodies, housing and communal authorities, any other bodies, organizations and institutions of the city of Kyiv (in particular, but not exhaustively, the following list: gas supply bodies, the relevant department of urban planning, architecture and regulation of land relations, the relevant energy supply company, communication authorities, water utility, etc.) on issues related to
1) registration of the ownership right that arose in me in relation to the PROPERTY, namely, to the APARTMENT under No. __, located in the residential building No. __, located on SICHNEVO POVSTVENIYA Street in the city of Kyiv;
2) managing this apartment and ensuring its viability;
3) organizing and carrying out repair works (including redevelopment, re-equipment) in the above apartment, with the right to obtain from the relevant authorities technical and any other necessary documentation for the above real estate object both before and after the completion of the repair;
4) ordering and obtaining all the necessary documents required by the applicable law for me to enter into further transactions on the alienation of this real estate (such as certificates of characteristics, certificates (other documents) on the condition of the real estate, absence of arrears of mandatory (due) payments, etc.
For this purpose, the representative is entitled to:
have free access to the above real estate;
to submit to the Kyiv City Bureau of Technical Inventory and Registration of Ownership of Real Estate concluded with my participation on "__" __________ ___ of the year the Sale and Purchase Agreement in respect of the above real estate for registration of ownership and receive it (with the relevant proper documents) after registration;
in cases known to the representative - to receive the documents necessary for me to alienate the said apartment documents, including - a certificate of characteristics, certificates (other documents) on the technical condition of the real estate, on the absence of debts on mandatory (due) payments, on the number of registered (registered) persons, etc;)
to conclude in my interests real estate and property insurance contracts, contracts for repair works, etc. on the terms and conditions that we have previously agreed upon and my representative is aware of, which does not require proof to third parties, a notary or other party(ies) to the transaction;
represent my interests in this regard in any institutions or enterprises regardless of their organizational form and subordination on issues related to ordering, certifying, obtaining the documents I need (including in the relevant district state administration, in local self-government bodies, executive bodies, housing and communal bodies, banking institutions, technical inventory bodies, notary bodies, any other organizations and institutions (including, but not limited to, the following list): district sanitary and epidemiological station, state fire protection authorities, gas supply authorities, relevant department of urban development, architecture and regulation of land relations, relevant structural unit of energy supplying institution or other similar authorities, State Archive, communication, water supply, labor protection authorities, etc. on issues related to the proper execution of the power of attorney and whose competence is related (but not limited) to the management of the above-mentioned real estate, repair and maintenance of the real estate object by the representatives
to represent my interests before any participants of civil relations (both individuals and legal entities) regarding the power of attorney where and when required by the current legislation of Ukraine, including before technical inventory bodies, internal affairs, etc., before individuals - when such a need arises, etc;
to submit on my behalf any requests, petitions, applications (both written and oral) related to the execution of the assignment, and to substantiate the requests and wishes set forth therein (including applications that will be necessary in my interests or under the requirements of applicable law);
submit any other documents necessary to resolve the issues referred to in this power of attorney;
to sign for me on all documents, without exception, that are issued, submitted, drawn up, executed, certified in connection with this power of attorney (in particular, to sign service agreements concluded within the framework of this power of attorney, acts of commissioning of facilities, applications, requests, petitions related to the execution of the matters specified in the power of attorney, etc;)
to obtain permits for construction works (repairs and alterations/remodeling of premises), expert opinions, certificates, technical documentation, decisions, approvals (consents) of competent authorities, extracts (including from state and unified registers), acts, copies of transactions concluded on my behalf, as well as any other documents related to the assigned task and arising from the content and essence of the power of attorney;
to organize and control the repair work, with the right to protect my interests in court in case of violation of the terms of the agreements;
pay for the services rendered and actions performed for me;
to raise questions about the proper fulfillment of the terms of the agreements, acceptance of the repaired (redesigned, converted) apartment into operation, obtaining title documents, with the right to perform all the necessary actions, submit and receive all the necessary documents;
to negotiate on my behalf on issues to be resolved in the course of the assignment;
to order an expert examination in cases where it is required by law or if it is necessary to protect my interests in the course of execution of this assignment.
The power of attorney is issued without the right of disposal, including alienation of the real estate mentioned herein.
The representative is entitled to delegate the powers granted by this power of attorney to third parties without further agreement with me.
The power of attorney shall be valid until October first, two thousand twenty-five, unless revoked by me earlier, and the representative shall be notified thereof in writing.
The power of attorney shall be valid in case of change of the representative's address of residence and/or his/her identification documents.
The content of Articles 240-245, 247-250 of the Civil Code of Ukraine regarding the concept of a power of attorney, its form, terms, assignment, termination of representation by power of attorney, revocation of a power of attorney and refusal of a representative to perform actions specified in the power of attorney has been explained to me by a notary.
