POWER OF ATTORNEY
City of Kyiv, the twenty-first day of February, two thousand twenty-four
LIMITED LIABILITY COMPANY [name] (registered office: Kyiv [address]; EDRPOU identification code: __) acting through its director [full name of citizen A.], who acts on the basis of the Charter, authorises [full name of citizen B.], who lives in Kyiv [address], to represent the interests of the company in connection with organising and carrying out repair works at the real estate object owned by the company, located according to the title document at: Kyiv, Zhylianska Street, building No. __.
For this purpose, the representative is entitled to:
- have free access to the specified real estate;
- prepare, submit and receive all documents needed for repair, redevelopment, reconstruction, approval of project documentation, obtaining permits, notifications, conclusions and commissioning of the facility;
- represent the company before state architectural and construction control bodies, DIAM, administrative service centres (CNAP), BTI, local self-government bodies, executive bodies, OSBB, the building manager, energy, gas, water and heat supply providers, communications bodies, labour-protection bodies, fire-safety bodies and any other bodies, institutions and organisations related to the carrying out and formalisation of repair works;
- order and receive technical, project, permit and other documentation relating to the real estate object, including the technical passport, technical conditions, project documentation, acts, conclusions, approvals, letters, certificates and other documents needed before the repair starts, during the works and after completion;
- enter into, amend and terminate contracts with design, construction, contractor, expert and other organisations needed to perform the repair works, as well as contracts for the supply of materials, technical supervision, author supervision, insurance and related services;
- sign statements, letters, petitions, acts, contracts, estimates, additional agreements and other documents related to the repair, redevelopment and reconstruction;
- receive and pay for works, services, mandatory payments and other expenses related to the preparation, carrying out and formalisation of the repair works;
- order expert inspections, technical inspections or other assessments if required by law or to protect the company's interests;
- perform all other actions necessary for the proper execution of this power of attorney.
This power of attorney is issued with the right to delegate powers to other persons and is valid until the twenty-first day of February, two thousand twenty-nine, unless terminated earlier in the prescribed manner, of which the representative will be notified in writing.
