Insurance Intermediary Information
This information is provided in accordance with Article 88 of the Law of Ukraine "On Insurance" No. 1909-IX dated 18 November 2021, Article 7 of the Law of Ukraine "On Financial Services and Financial Companies" No. 1953-IX, and the Regulation approved by NBU Board Resolution No. 2 dated 3 January 2025. 2. Status and powers of the intermediary
Individual entrepreneur Ihor Balaban is an insurance intermediary — a sub-agent who sells insurance products in the name and in the interests of insurers under a sub-agency agreement with the insurance agent:

Under this agreement the sub-agent is authorised to carry out the following distribution activities (Article 71(1) of the Law of Ukraine "On Insurance"):
- advertising, marketing and other preparatory activities aimed at the conclusion of insurance contracts, providing information on the terms of an insurance product, comparative analysis and advice based on the client's criteria and needs;
- offering insurance products and advising the client on the conclusion of an insurance contract, other preparatory work;
- concluding and amending insurance contracts, preparing documents and calculations relating to the payment of the insurance premium.
The sub-agent is not authorised to collect insurance premiums from clients. The insurance premium is paid by the policyholder directly to the current account of the relevant insurer.

4. Individual consultation
The client may obtain from the intermediary an individual consultation on the terms of an insurance product and a recommendation as to which insurance product best meets the client's requirements and needs. To receive a consultation, call +38 (044) 232-50-60 or e-mail write an email. Before giving a recommendation, the intermediary ascertains the client's insurance requirements and needs.

These insurers provide services within the classes of insurance covered by their licences (voluntary and compulsory insurance, including accident insurance, medical insurance — among them medical insurance for foreign nationals temporarily staying in Ukraine — motor insurance, liability insurance and travel insurance). The full list of each insurer's services is available on its website. The insurers' details and licences can be verified in the State Register of Financial Institutions via the NBU information system: kis.bank.gov.ua.
6. Qualifying holdings
The intermediary does not hold a qualifying holding in any insurer, and no insurer holds a qualifying holding in the intermediary.
7. Remuneration of the intermediary
The intermediary's services are free of charge for the client: the client pays no fees to the intermediary. The intermediary receives remuneration for the distribution of insurance products from the insurers (through the insurance agent) under the sub-agency agreement; this remuneration is included in the insurance premium under the insurance contract.
8. Other payments by the client
Apart from the insurance premium paid directly to the insurer, the client makes no other payments to the intermediary. If the insurance contract of a particular insurer provides for other payments by the client after its conclusion, information about them is set out in the relevant insurance contract and in the insurance product information document before it is concluded.

- you may contact the insurer under your contract at its registered office (see the table in section 5) or via the complaints section of its website — out-of-court handling of complaints by the insurer is free of charge;
- you may contact the National Bank of Ukraine as the financial market regulator: bank.gov.ua/en/consumer-protection; postal address: 9 Instytutska St., Kyiv, 01601, Ukraine;
- you may apply to court in accordance with the legislation of Ukraine.
