Power of attorney

I am aware of the general requirements, compliance with which is necessary for the validity of the transaction, fully realizing the significance of my actions and in accordance with the free expression of my will, which corresponds to my inner will,
in accordance with the requirements and pursuant to Articles 244 and 1007 of the Civil Code of Ukraine on the basis of an oral power of attorney concluded with the attorney,
in the presence of Alla Mykhailivna Kvitka, a notary of the Kyiv City Notary District, whom I chose as a notary independently and to whom I applied voluntarily, at the address: Kyiv [address], where the notary was invited by me personally due to my advanced age, by this power of attorney

in the relevant authorized bodies, institutions, organizations, in particular - in state authorities and local self-government bodies, executive bodies, housing and communal bodies, internal affairs bodies, pension bodies and/or social protection and insurance bodies, at the post office, in a banking institution known to the representatives, in heat, gas energy supply, telecommunication, land management authorities, etc.) on issues related to the receipt of my PENSION (other social benefits) and ensuring the proper condition of the HOUSE at No. __, located on Sadova Street in Irpin, Kyiv region, and the LAND LOT on which the above-mentioned house is located.
For this purpose, the representatives are entitled to:
 submit on my behalf any documents related to the execution of the power of attorney (including statements that will be required by my interests as a recipient of pensions and other social benefits and the owner of the above-mentioned real estate) and substantiate the information, requests and wishes set forth therein;
 have free access to the above-mentioned real estate;
 to present his/her arguments and considerations on all issues arising in the course of the power of attorney execution, as provided for by law;
 to sign for me on documents that will be submitted (issued) in the course of the power of attorney execution (including signing for receipt of pension or other monetary payments);
 to receive a pension or other social benefits accrued to me, regardless of where and by whom such payments will be made;
 raise questions and resolve them on the merits regarding gas, water, heat supply of the residential building, normal functioning of telephone communication, normal operation of other telecommunication systems, unimpeded use of the residential building and land, etc;
 to pay for the services provided on my behalf and/or to pay state duty or other necessary payments;
 perform other actions provided for by the current legislation of Ukraine for this type of authorization, and which will be necessary and appropriate for the correct and efficient execution of the assignment.
Depending on the circumstances, the representatives may exercise the powers granted to them under the power of attorney both jointly and separately (independently) from each other. In the latter case, the one who actually performs the actions stipulated in the power of attorney will be considered my representative.
The power of attorney is issued with the right to delegate powers to third parties and shall remain valid until December 31, two thousand twenty-five, unless terminated earlier in accordance with the requirements of civil law, of which I will notify the representatives in writing.
The content of Articles 240-245, 247-250 of the Civil Code of Ukraine regarding the concept of a power of attorney, its form, terms, assignment, termination of representation by power of attorney, revocation of a power of attorney and refusal of a representative to perform actions specified in the power of attorney has been explained to me by a notary.
