Power of attorney

I am aware of the general requirements necessary for the validity of this transaction, fully realizing the significance of my actions and in accordance with my free will,
in accordance with Articles 244 and 1007 of the Civil Code of Ukraine on the basis of an oral power of attorney concluded with the attorney,
in the presence of Alla Mykhailivna Kvitka, a notary of the Kyiv City Notary District, whom I chose independently and to whom I applied voluntarily, at the address: Kyiv [address], where the notary was invited by me personally due to my advanced age, by this power of attorney
I hereby authorize
[full name of citizen B.], who resides in Kyiv [address], and/or
[full name of citizen V.], who resides in Kyiv [address],
to represent my interests in relevant state authorities, local self-government bodies, their executive bodies, housing and communal enterprises and organizations, the National Police of Ukraine, territorial bodies of the Pension Fund of Ukraine, social protection bodies, at the post office, in banking institutions, at suppliers of heat energy, natural gas, electricity, water and electronic communications services, before state cadastral registrars, state registrars of rights to immovable property, in administrative service centres and other authorised bodies in matters related to receiving my accrued pension and other social benefits and ensuring proper condition of the residential house No. __, located on Sadova Street in Irpin, Kyiv region, and the land plot on which the above-mentioned house is situated.
For this purpose, the representatives are entitled to:
• submit on my behalf any documents relating to the performance of this power of attorney (including applications that will be required by my interests as a recipient of pension and other social benefits and the owner of the above-mentioned real estate) and substantiate the information, requests and wishes set out in them;
• receive certificates, extracts, acts, invoices, receipts and other documents related to the performance of this power of attorney;
• have free access to the above-mentioned real estate;
• express their arguments and considerations on all issues arising in the course of the power of attorney performance, as provided for by law;
• sign documents on my behalf that will be submitted (issued) in the course of the power of attorney performance (including signing the receipt of pension or other monetary payments);
• receive the pension accrued to me or other cash social payments, regardless of where and by whom such payments will be made;
• raise issues and resolve them on the merits regarding the supply of heat energy, natural gas, electricity, water, the normal functioning of telephone communication and other electronic communications systems, and the unhindered use of the residential house and land plot, etc.;
• make payments on my behalf for services rendered, pay state duty, administrative fee or other necessary payments;
• perform other actions provided for by the current legislation of Ukraine for this type of authority, and which will be necessary and appropriate for the correct and efficient performance of the assignment.
Depending on the circumstances, the representatives may exercise the powers granted to them under the power of attorney jointly or separately (independently) from one another. In the latter case, the one who actually performs the actions provided for by this power of attorney will be considered my representative.
The power of attorney is issued with the right to delegate powers to third parties and shall remain valid until December 31, two thousand twenty-seven, unless terminated earlier in accordance with the requirements of civil law, of which I will notify the representatives in writing.
The content of Articles 244-250 of the Civil Code of Ukraine regarding the concept of a power of attorney, its form, terms, delegation, termination of representation by power of attorney, revocation of a power of attorney and refusal of a representative to perform actions specified in the power of attorney has been explained to me by the notary.
