I, [full name of citizen A.], passport series ____ No. _____________, issued by _______________________________ on "____" _____________ ______ year, taxpayer identification number ____________, residing in Kyiv [address], acting freely and with full awareness of the meaning of my actions, pursuant to an oral mandate agreement and Article 1007 of the Civil Code of Ukraine, hereby authorise [full name of citizen B.], who resides in Lutsk [address], to represent my interests in relation to the vehicle belonging to me, an AUDI A8, chassis No. ERTYUPK4E96N033961, black in colour, as evidenced by the vehicle registration certificate series ____ No. _________, issued by the territorial service centre of the Ministry of Internal Affairs of Ukraine in Kyiv on "____" __________ _____ year and registered in my name at the territorial service centre of the Ministry of Internal Affairs of Ukraine in Kyiv on "____" __________ ____ year, with the right to:
- represent my interests before the MIA service centres of Ukraine, the National Police of Ukraine, insurers, notaries, commission shops, service stations, repair, storage and evacuation facilities, and other enterprises, institutions and organisations regardless of ownership or subordination;
- submit, sign on my behalf and receive applications, explanations, motions, forms, acts, notices, requests and other documents, sign for me, receive the vehicle registration certificate, temporary registration certificate, number plates, including transit plates, and other documents relating to the vehicle;
- carry out registration, re-registration, deregistration and changes to the registration data of the vehicle, undergo technical inspection (technical control) where required by law, organise maintenance, repair, diagnostics, parking, storage, evacuation and transport of the vehicle, including driving it while it is being moved, provided the attorney has the relevant category of driving entitlement;
- order and pay for services related to the operation of the vehicle, and pay taxes, fees, administrative charges and other compulsory payments related to its registration, maintenance, operation, storage and use;
- conclude, amend and terminate compulsory motor third-party liability insurance contracts and other insurance contracts, receive insurance policies, certificates and other documents related to an insured event, and claim and receive insurance compensation;
- starting from "___" ____________ 20__ (after the existing restrictions are lifted), dispose of the said vehicle, including selling, exchanging, leasing, lending, pledging it as security for my own obligations, entering into sale, exchange, lease, loan and pledge agreements, and also represent my interests before notary bodies, commission shops, insurers and other enterprises, institutions and organisations on all matters connected with the sale and formalisation of the vehicle;
- perform other legally significant acts necessary for the proper operation and maintenance of the vehicle, within the limits of the current legislation of Ukraine.
For the performance of the granted powers, the representative may use and operate the vehicle for its intended purpose.
This power of attorney is issued without the right to delegate these powers to third parties, and remains valid until "___" ____________ 20__ year or until its cancellation in the manner prescribed by civil law.
