POWER OF ATTORNEY

I, [full name of citizen A.], passport series ____ No. _____________, issued by _______________________________ on "__" ____________ ___, taxpayer registration number ____________, residing at [address], acting voluntarily and pursuant to Articles 244-250 of the Civil Code of Ukraine, hereby authorise [full name of citizen B.], who resides at [address], to represent my interests in relation to apartment No. ___ in building No. __ at [address], the conclusion, amendment, extension and termination of the lease agreement for this apartment, and to take all actions necessary for letting the apartment, its proper use and obtaining documents.
For this purpose, the attorney may:
- represent my interests before a notary, a centre for administrative services (CNAP), the state registrar of rights to immovable property and their encumbrances, the BTI or any other technical inventory provider, local self-government bodies, executive bodies, residence registration authorities, tax authorities, the condominium association (OSBB), the managing company, housing maintenance organisations, utility providers, communication providers, insurers, appraisers and any other institutions and organisations if needed to perform this power of attorney;
- submit and receive applications, requests, certificates, extracts, information certificates, a technical passport, a property characteristics certificate, acts, decisions, registration documents, duplicates and any other documents needed for the conclusion and performance of the apartment lease agreement;
- order and receive technical inventory, a technical passport, property valuation, certificates and other documents needed for concluding or extending the agreement, and for state registration of rights if such registration is required;
- sign applications, handover and acceptance acts, receipts, declarations, lease agreements, supplementary agreements, apartment inspection reports and any other documents connected with this power of attorney;
- conclude, sign, amend, extend and terminate lease agreements, determine the lease term, rent amount, payment procedure, distribution of utility costs, deposit and other essential terms at my own discretion, and receive the money due to me;
- file documents for state registration of rights, if such registration is required, and receive extracts from the State Register of Rights to Immovable Property, decisions, refusal notices or other documents issued after the applications are considered;
- register or deregister the place of residence of persons living in the apartment, enter into and sign related agreements with the condominium association, the managing company and utility providers, and resolve issues relating to utility charges, access to the apartment and utility connections;
- inspect the apartment, hand over and receive keys, prepare handover and acceptance acts, inspection reports, property lists and other documents related to leasing the apartment;
- pay the administrative fee, notarial charges, the cost of technical inventory, valuation, extracts, services and any other mandatory payments connected with this power of attorney;
- challenge refusals, actions or omissions of officials and perform any other acts necessary for the full performance of this power of attorney.
This power of attorney is issued without the right to delegate powers to third parties. It shall be valid until "___" ____________ 20__ unless revoked by me earlier. It remains valid if the representative changes their residential address and/or passport.
The content of Articles 244-250 of the Civil Code of Ukraine has been explained to me by a notary.
