POWER OF ATTORNEY

I, [full name of citizen A.], passport series ____ No. _____________, issued by _______________________________ on "__" ____________ ___, taxpayer identification number ____________, residing at [address], acting voluntarily and fully aware of the meaning of my actions, pursuant to Articles 244-250 of the Civil Code of Ukraine, hereby authorise [full name of citizen B.], who resides at [address], to represent my interests in matters of redevelopment, re-equipment, repair, extension and installation of a separate entrance group for apartment No. __ in building No. __ on Krakivska Street in Kyiv, as well as in matters of obtaining all documents, approvals and decisions required for the performance and formalisation of such works.
For this purpose, the attorney may:
- represent my interests before a centre for administrative services (CNAP), local self-government bodies, bodies dealing with urban planning and architecture, state architectural and construction control bodies, the BTI or any other technical inventory entity, the condominium association (OSBB), the managing company, housing maintenance organisations, utility providers, tax authorities and other institutions if needed to perform this power of attorney;
- submit and receive applications, notifications, petitions, declarations, permits, approvals, conclusions, acts, decisions, technical documentation, technical conditions, project documentation, estimates, plans, schemes, a technical passport and any other documents needed for the performance and formalisation of the redevelopment, re-equipment, extension, repair and installation of a separate entrance group;
- order, agree and receive project, technical and as-built documentation, surveys, conclusions, reports, measurements, floor plans and other documents connected with such works;
- enter into, sign, amend and terminate contracts for design, technical inspection, repair and construction works, author supervision and technical supervision, handover and acceptance acts, work completion acts, addenda and other related documents;
- file documents to obtain a notification, declaration, permit or other permit documents and approvals if these are required by law for such works, and receive decisions on registration, refusal or any other outcome documents;
- submit statements and documents that may be required to confirm the consent of a co-owner, spouse or any other person if such consent is required by law for the works;
- order technical inventory and receive an updated technical passport after the works are completed, as well as other documents connected with the update of the apartment's technical records;
- file documents for state registration of changes relating to the immovable property object, if such registration is required by law, and receive extracts, decisions, notices, refusals and other registration documents;
- pay administrative fees, notarial fees, and the cost of design, technical, inventory, construction and utility services, as well as any other mandatory payments connected with this power of attorney;
- challenge refusals, actions or omissions of officials and authorities if they obstruct performance of this power of attorney, and exercise all rights of the apartment owner needed to protect my interests;
- perform any other acts necessary for the full performance of this power of attorney.
This power of attorney is issued without the right to delegate powers to third parties. It shall be valid until "___" ____________ 20__ and in any event for no longer than three years from the date of notarisation, unless revoked earlier.
