I, [full name of citizen A.] (citizen of Ukraine; born "__" ____________ _____ year; on the day of issuance of the power of attorney - passport _________, issued by ____________________ "__" _______________ ___ year; registration number of the taxpayer's account card - ___________), residing at the address: Kyiv [address], aware of the general requirements, compliance with which is necessary for the validity of the transaction, fully realizing the significance of my actions and in accordance with the free expression of my will, which corresponds to my inner will, in accordance with Articles 244 and 1007 of the Civil Code of Ukraine and on the basis of an oral mandate agreement concluded with the attorney, in the presence of Alla Mykhailivna Kvitka, notary of the Kyiv City Notary District, whom I have chosen as a notary on my own and to whom I have applied voluntarily, by this power of attorney I authorize [full name of citizen B.], who lives in Kyiv [address], and/or [full name of citizen V.], who lives in Kyiv [address], and/or [full name of citizen G.], who lives in the city of Kyiv [address], and/or [full name of citizen D.], who lives in the village of Rusaniv, Brovary district, Kyiv region [address], to represent my interests in relation to the vehicle owned by me, namely the VOLKSWAGEN car, model TRANSPORTER, year of manufacture 2001, yellow colour, having chassis (body, frame) No. ____________, registration number - _________, registered in my name in the Traffic Police Department of the Main Office of the Ministry of Internal Affairs of Ukraine in Kyiv on "__" _____________ _____ year.
For the proper exercise of the powers granted by this power of attorney, the representatives shall be granted all the rights required by the current legislation related to ensuring the proper operation of the vehicle, in particular with regard to:
- representation before territorial service centres of the Ministry of Internal Affairs of Ukraine, the National Police of Ukraine, insurers, notaries, maintenance, repair, storage and towing service providers, and other enterprises, institutions and organisations regardless of ownership or subordination;
- submitting and signing on my behalf applications, explanations, requests, acts, notices and other documents; receiving certificates, copies, duplicates, the vehicle registration certificate, a temporary registration certificate, license plates, including transit plates, and other documents related to the car;
- carrying out state registration, re-registration, deregistration and amendments to the registration data of the car; determining the parking place of the car; undergoing state technical control (technical inspection); arranging maintenance, repair, diagnostics, storage, towing and transport of the car, including while driving it during movement, provided the representative has the relevant driving licence category, including outside Ukraine;
- ordering and paying for services related to the operation of the car, as well as paying taxes, fees, administrative and other mandatory payments related to its registration, maintenance and storage;
- entering into motor third-party liability insurance contracts and other insurance contracts, receiving insurance policies, certificates and other documents related to an insured event;
- performing all other legally significant actions related to the proper operation and maintenance of the car.
For the exercise of the powers granted, the representatives shall have the right to use and operate the car in accordance with its intended purpose.
Depending on the circumstances, the representatives may exercise the powers granted to them under the power of attorney both jointly and separately, independently of each other.
This power of attorney is issued without the right to alienate the car and without the right to delegate powers to third parties, and remains valid until "___" ____________ 20__ year or until revoked in accordance with the procedure established by civil law, of which the representatives (the representative) will be notified by me in writing.
The content of Articles 240-245, 247-250 of the Civil Code of Ukraine regarding the concept of a power of attorney, its form, terms, assignment, termination of representation by power of attorney, revocation of a power of attorney and refusal of a representative to perform actions specified in the power of attorney has been explained to me by a notary.
