I, [full name of citizen A.], passport series ____ No. _____________, issued by _______________________________ on "____" _____________ ______ year, taxpayer identification number ____________, residing in Kyiv [address], acting freely and with full awareness of the meaning of my actions, pursuant to an oral mandate agreement and Article 1007 of the Civil Code of Ukraine, hereby authorise [full name of citizen B.], who resides in Chernihiv [address], to represent my interests in relation to the vehicle belonging to me, JAGUAR X-TYPE, passenger saloon, manufactured in 2006, yellow in colour, chassis (body, frame) No. ___________, registration number _______, as evidenced by the vehicle registration certificate series ____ No. __________, issued by the territorial service centre of the Ministry of Internal Affairs of Ukraine in Kyiv on "___" ____________ _____ year, with the right to:
- represent my interests before the Main Service Centre of the Ministry of Internal Affairs of Ukraine, territorial service centres of the Ministry of Internal Affairs of Ukraine, the National Police of Ukraine, insurers, notaries, service, repair, storage and evacuation stations, and other enterprises, institutions and organisations regardless of ownership or subordination;
- submit, sign on my behalf and receive applications, explanations, motions, forms, acts, notices, requests and other documents; sign documents for me; receive certificates, copies, duplicates, the vehicle registration certificate, temporary registration certificate, number plates, including transit plates, and other documents relating to the vehicle;
- carry out state registration, re-registration, deregistration and changes to the vehicle registration data, undergo mandatory technical inspection (technical control) where required by law, organise maintenance, repair, diagnostics, storage, towing, evacuation and transport of the vehicle, including driving it while it is being moved, provided the attorney has the relevant driving entitlement category;
- order and pay for services related to the operation of the vehicle, and pay taxes, fees, administrative charges and other mandatory payments related to its registration, maintenance, operation, storage and use;
- conclude, amend and terminate compulsory motor third-party liability insurance contracts and other insurance contracts, receive insurance policies, certificates and other documents related to an insured event, and claim and receive insurance compensation;
- perform other legally significant actions necessary for the proper operation and maintenance of the vehicle, within the limits of the current legislation of Ukraine.
For the performance of the granted powers, the representative may use and operate the vehicle for its intended purpose.
This power of attorney is issued without the right to dispose of the vehicle and without the right to delegate these powers to third parties, and remains valid until "___" ____________ 20__ year or until its cancellation in the manner prescribed by civil law.
The content of Articles 244-250 and 1007 of the Civil Code of Ukraine has been explained to me.
